Certification Forms and Documents

The following documents are available for you to download and use in submitting an application or updating your certification with Nature’s International Certification Services. Keep in mind that a grower organics system plan (OSP), field history sheet, maps, organic certification affidavit, and the appropriate fees must accompany every file. A brief explanation of each section follows.

These are the Organic System Plans you must fill out for certification.

CO2a Grower OSP must be filled out by every applicant the first year.
CO2b Grower OSP Update is filled out the years following initial certification.

The following are specific production options (add-ons) for certification. Please download fee sheet to view fees for add-ons.

Use this fee sheet to figure the amount you must send in with your application. Applications will not be processed without payment.

The following is a set of record keeping documents for you to use and keep in your files. In some cases you will need to send in certain documents with your file to verify your compliance with the Rule.

Field History sheet - must accompany every file
Prior Land Use Declaration Form (PLUD) - must be filled out and sent in when land has not been managed by applicant for at least 3 years
Newly purchased land or rented land verification - as applicable, similar to PLUD above
Seed verification form - use to document purchased seeds and organic seed search efforts   
Field activity log - document all field activities (tillage, planting, cultivation, harvest, etc.)
Input use record - excellent for listing all inputs (fertility, pest/disease control, livestock health)
Compost production record - to track and document compost you make
Neighbor notification letter - a sample letter to send to neighbors to let them know you are producing organically
Verification of adjoining land use - for a neighbor to fill out verifying the use of their land
Buffer crop usage - to record deposition of buffers
Crop harvest record - to record harvests of crops
Crop harvest and storage record - to record harvests and storage of crops
Clean transport affidavit - to record cleanout of truck/transport
Clean Equipment Log - for use when equipment is used on conventional and organic crops
Split operation or parallel production crop record - for use when conventional or transitional and organic crops are grown on same farm
Audit control summary - to consolidate information about all production and sales activities
Complaint log - keep track of complaints about your operation
Transaction Certificate Authorization - for use when buyer of organic products requests additional verification
